When you are in a hurry, it is always better to have your forms completed before you come for your appointment. That way, the information you give to us will be more complete. We can do what you are requesting without us having to call you continuously to find out. For your convenience, we have these forms in downloadable files.

If you are a new client, let us thank you for your confidence in us. The new client form gives us the information we need to put you and your pets' information in the computer to get your record started.

If you are dropping your pet off for medical services, the drop off form will help us understand why the pet is here with us and your instructions to us. It is very important to leave us a phone number for the day where you will be reachable if we have any questions or problems.
 
 
 
 
 
 
 
For boarding pets, we require several questions to be answered on the boarding form before letting pets into the boarding area. The boarding form will let us know things like medications that are needed to be given, special diets that need to be fed, or if you want a bath or grooming before you pick your pet up.

Should your pet be in for surgery, our surgery form will give you the options of having important optional services performed at the same time for the safety and well being of your pet while in our care.
 
 
For patients that need a teeth cleaning or extractions, you can fill out the dental surgical form before your arrival. We recommend preanesthetic blood testing and dental X-rays. Often, we do not know exactly what we will need to do until the teeth get cleaned so that we can evaluate the extent of dental disease. Fees for many of the services that might be necessary along with your permission to proceed are included on this form.
If you are coming in for an appointment with Dr. Gotthelf for skin or an ear problem, it is very important for us to have a skin disease form filled out. That gives the doctor your pet's history pertaining to things that can affect the skin and ears. That gives us a good start at determining the potential causes of your pet's skin and ear disease.
